Whew, this is an impressive index! I started keeping somewhat of an index just of the URLs to multimedia, but soon gave up as I didn't have the umph to check out all of my past posts, nor the inclination to keep the index current.
I am guessing you put this index together manually, i.e. without any assistance from some cool tool? Especially if that's the case, congratulations on your accomplishment!
I like the idea of having an index or some form of organization for finding blog posts/topics. With that said, I rarely use them on other people's blogs, preferring to attempt to stay current with reading (though that does not happen as often as I'd like!)

I'm watching my Google Analytics. So far this post has done well in popularity. I suspect that's to do with factors not related to day to day visitors though. It may be as you alluded and as Sue Waters also prescribed, that no one will use it. That remains to be seen.
Like you, I find a need for a blog post index. In this respect I've already found this invaluable - navel gazing some would call it. But when I want to find a link in a comment someone left or a reference to a conversation and I've forgotten where or when it was posted, the index makes it far easier to get onto the trail.
I put it together manually - a bit of time - not days, but now it's up it'll be no bother to maintain.
There were a lot of features in Blogger that made it easy. The Labels Index was sweet to compile. I did it more or less in one copy and paste with a bit of formatting.
Believe it or not, Technorati was also very useful when I pulled across my total blog list.
Don't tell Sue Waters, but I also used Word. Knowing what I know about html, there was no problem. But for someone who doesn't know the idiosyncracies of Word, there could be major issues using it the way I did.
On reflection, well chosen labels for posts can provide a very good component for a simple blog index. There are widgets that can utilise these with the least effort on the part of the blogger. As Sue Waters pointed out to me, a widget like this is (not yet) available in Blogger. Time enough though.

Back again scoping out your index for a second look. I've been mulling over whether or not to refine my tags in order to be more useful/global, or do away with tags altogether.
Somewhere along the way, perhaps after passing a certain point in the number of my posts, it no longer seemed necessary to tag them. I've started simply using my blog's search feature to find specific posts or topics.
I'm curious to know how you are keeping up with making entries to your index.

I find maintaining the index is very easy. It's just one of these things I do when I write a post, along with all the other things that are essential to good blogging, such as checking that the post works in other versions of other browsers - that can take more time than the indexing of the post.
You mention tags and the use of these. In this index, I have used the tags as a separate index list. It's actually quite useful, for it lists ALL posts when the tag label is selected.
I have never discussed using the 'Search' facility. Some people don't like it. Frankly, I have used it on occasions and found it to be quite useful.
Somehow, I think the issue of indexing is really quite complex, for different people use indices differently. Not everyone likes indexing and not everyone knows how to use an index. By the same token, not everyone knows how to use a search facility properly either. I discussed this quite extensively on Tony Karrer's post with Sue Waters and Tony. Check out the link.
